As much as I'd like to point fingers, I'd say the whole system is prone to derailments such as this. Any supply officer in the CAF has had the potential to employ a few of these tactics. There are not many checks to keep them on track.

I'm pretty sure anyone in those positions would know of the flaws at their disposal to abuse.
