In my opinion, Red Wedding is for sure the most dirty deed and Freys deserved 10oz melted lead in the mouth ass.

As regards the 3rd question I suppose Catelyn Stark is guilty in unleashing war and extermination of the Stark House. It is she that captured Tyrion and accuse him in attempt on Bran's life. This unwise act had grave consequences as we remember. The second big fault of this brainless person was let Jaime Lannister to go for exchange his life for Arya and Sansa.
Sure thing that she isn't a half as dirty as almost every from Lannister House but hell is full of good meaning and wishings.
